Why is My French Bulldog Regurgitating After Soft Palate Surgery?

Dr. Kraemer, we adopted our French bulldog, Cersi from a breeder in FL as a "retiree", she was 3 years old and had already had 3 liters.  We knew after the initial meet that she would require soft-palate surgery (showed all the typical signs).

Why My French Bulldog Is Regurgitating?

It has been 2+ months now and she is experience regurgitation on a daily basis...several times a day. 

We are feeding her a

  • low-fat, dehydrated whitefish food and splitting it up into 3 smaller meals.
  • I have added raw goats milk as a "bedtime snack" so her belly is not empty overnight.

BOAS Breeds Ownership

We have had flat-faced dogs in the past...Frenchie, English & a Pug.

From what you described, the regurgitation started after the soft palate surgery, and she was fine before. That timing  is critical to my answer.

What is Regurgitation?

For those of you confusing it with reflux and vomiting, regurgitation is basically a food U-turn before reaching the stomach.

The food goes down the esophagus, changes its mind, and comes right back up. Because of that, it looks undigested.

In bulldogs, the most common reason for this is a condition called megaesophagus.

What is he difference between regurgitation and vomiting?

REURGITATINION

  • Regurgitation is passive
  • No abdominal effort
  • The food looks whole
  • Usually instant or right after a meal

VOMITING

  • Active abdominal contractions
  • Your bulldog may look nauseated beforehand
  • Th material may contain bile or partially digested food

What is Megaesophagus?

Instead of being a firm, active tube that pushes food straight into the stomach, a section of the esophagus becomes weak and stretched out.

Imagine a smooth slide at a playground. When the slide is straight and firm, the child goes right down. But if the middle of the slide sags and forms a dip, the child gets stuck in that low spot instead of reaching the bottom.

In bulldogs, this often happens because of chronic acid reflux.

The stomach acid reflux irritates the esophagus over time, causing inflammation (esophagitis). That inflammation weakens the muscles, and eventually the esophagus loses its ability to move food properly.

How Does Esophagitis Happens?

In French bulldogs, the number one culprit is Brachycephalic Obstructive Airway Syndrome, or BOAS.

Those bulldogs struggle to breathe, generating a high abdominal pressure and high vagal tone leading to vomit and reflux. Over time, that repeated irritation causes trouble.

That is why, after the elongated palate surgery, we would typically expect reflux problems to improve, not suddenly appear.

So if symptoms begin after surgery, the surgical area may need to be reassessed, and if all looks as it should other rule outs and diagnosis should be conducted.

Other causes of chronic reflux include hiatal hernia and GERD.

What Else Can Cause Megaesophagus?

OBSTRACTION OF THE ESOPHAGUS

Sometimes there is a physical blockage, such as a

  • tumor
  • foreign body
  • scar tissue
  • narrowing the esophagus (stricture).

VASCULAR ANOMALY

There is also a congenital condition called vascular ring anomaly (VRA). In that situation, an abnormal blood vessel forms a ring around the esophagus near the heart base and compresses it.
The most common version is a persistent right aortic arch (PRAA).

 But remember this is an inborn condition that start at a very young age.

NUROLOGICAL CONDITION

Neurologic conditions such as myasthenia gravis can also affect esophageal function but are very rare.

What Testing To Run On Fr. Bulldog Regurgitating ?

If the timing clearly lines up with the surgery, the palate and larynx should be reevaluated.

⚠️Warning: That requires anesthesia.

We also need to  test for the other causes to make sure we are not missing something else.

What Regurgitations Tests Can Be Done Without Anesthesia?

  1. Radiographs of the neck, chest, and abdomen are a good starting point.
  2. A contrast swallow study using barium mixed with food can show where the food is getting stuck. ⚠️Warning: the key rule is never to force barium (with a syringe as an example).
  3. An ultrasound performed by an experienced ultrasonographer can also provide useful information.

What Regurgitations Tests Require Anesthesia?

  1. Endoscopy
  2. CT imaging,
  3. Direct visualization of the surgical area all require anesthesia.

How To Manage French Bulldog Regurgitation?

First, we diagnose the underlying cause. That is the long-term solution.

In the meantime, management becomes a bit of a lifestyle adjustment.

  1. Meals should be elevated.
  2. Vertical Feedings: Some bulldog need to be fed in an upright position (Baily Chair).
  3. After eating, they should remain vertical for about ten minutes (let gravity do its thing).

In some cases, a Bailey chair for upright feeding becomes part of the furniture.

4. Food type & texture may need to be adjusted. Sometimes blended prescription diets work better. Sometimes canned, sometimes moist "meatball" like texture.

5. Medication: some dogs benefit from medications that help improve esophageal motility (promotility like Viagra). If reflux is part of the problem, short-term acid suppression may be prescribed.

What Are The Risks of Regurgitation?

Chronic regurgitation can lead to

  • Aspiration Pneumonia: when the regurgitated food enters the lungs. That is the serious complication we work hard to prevent.
  • Weight Loss
  • Nutritional deficiencies

Good quality nutrition and appropriate supplementation  may be necessary during recovery.
